PART 2 - TRAINING CAMP SELECTION
In preparation for the main competition day, athletes are invited to participate in at least one training camp. At each training camp, athletes will meet their team and coach and take part in athletic workshops. All training camps are free of charge.

Please select at least one training camp to attend:

	Sat Sept 25 (9am-1pm): Valleys P.S - 1235 Mississauga Valley Blvd.
	Sat Sept 25 (2pm-6pm): Valleys P.S - 1235 Mississauga Valley Blvd.
	Sun Sept 26 (10am-2pm): T.L. Kennedy S.S - 3100 Hurontario St.

PART 3 - SPORT SELECTION
On competition day (Sun Oct 3), each athlete will take part in two (2) sports. Please indicate your first, second, and third choices of sports. Although we cannot guarantee an athlete's place in any one sports bundle, we will consider preferences when assigning an athlete to a team. Repeated selections will not be accepted.

In consideration of my child’s participation in this program, I hereby waive, release and discharge any and all claims for damages I may have, or which may hereafter accrue to me, as a result of my participation in this activity. This release is intended to discharge in advance the instructors, promoters, sponsors, organizers, project managers, of this activity and any involved public school entity (and their respective agents and employees) including but not limited to MY Games – Mississauga Youth Games, their employees, agents and directors, from and against any and all liability, which may arise out of negligence or carelessness on the parts of the persons or entities mentioned above. I understand that this program may be photographed or videotaped, and MY Games – Mississauga Youth Games has my permission to use the photographs, videotapes, and/or audiotapes for the purpose of promoting the work and mission of their organization.
